Please be advised 9/13/2023 was my last day with Village Medical.  I have chosen to resign in order to pursue a new career opportunity.

If you would like a copy of your medical records this can be requested through the village app, or by calling the medical records department at 888-625-2317 and filling out a release of healthcare information form.
